This active outreach role involves building trust, responding during crisis, supporting safety and connection, and walking alongside young people as they set their own goals.

In this role, you will:

• provide outreach and case management support to young people with complex needs
• help young people respond to immediate needs, reduce harm and improve safety
• support connection to family, kin, community and Country, where wanted and safe
• attend stakeholder meetings
• speak up for young people’s views, rights, choices and access to resources
• complete case notes, incident reports and other records needed for safe practice
• take part in supervision, debriefing, reflection and team planning
